Certificate Program

Requirements

Official High school transcript, official GED transcript, or “Ability to Benefit Test” from Spirit & Life Seminary. Offered in collaboration with Spirt & Life Seminary, a COGOP Institution Program: Ministry and Theological Studies (English & Spanish Cohorts)

  1. Fill out the application at SLSEM.EDU under the “Apply Now”
  2.  Pay the non-refundable $50 application fee to Spirit and Life Seminary.
  3. Submit a copy of your official high school (GED or college) transcript to info@slsem.edu or contact Spirit and Life Seminary to take the “Ability to Benefit Test.” ATB test fee is $25 (non-refundable). Documents may also be sent by regular mail.
  4. One letter of recommendation from someone other than a family member.
  5. Sign and date the Transferability of Credit Disclosure form.
  6. Complete, sign and date along with a Spirit & Life Seminary representative the PreEnrollment Checklist.
  7. Complete, sign and date along with a Spirit & Life Seminary representative the Enrollment Agreement.

Requirements

Official College transcript

Fill out the application at SLSEM.EDU / under the “Apply Now”

  1. Pay the non-refundable $60 application fee to Spirit and Life Seminary.
  2. Have your institution send a copy of your official college transcript to info@slsem.edu
  3. One letter of recommendation from someone other than a family member.
  4. Sign and date the Transferability of Credit Disclosure form.
  5. Complete, sign, and date the Pre-Enrollment Checklist.
  6. Complete, sign, and date the Enrollment Agreement.

Requirements

Submission of an official bachelor’s degree transcript from an accredited institution in any discipline, or an official certificate transcript from Spirit & Life Seminary.

Graduate Programs (cont.)

  1. Fill out application at cogopamd.org under the “ADMISSIONS” option; select “Master’s Program – application.”
  2. Submit non-refundable $75 application fee to AMD.
  3. Request official transcript from each accredited institution of higher learning documenting the completion of a degree. Transcripts must be sent from the institutions to AMD either by e-mail (amd@cogop.org) or regular mail.
    Note: Graduates from non-American institutions will need to have their transcript evaluated. Transcripts form Candian institutions do not need to be evaluated.
  4. Submit an autobiography describing your personal background (birthplace, family life, employment), church affiliation/credentialing, Christian experience (conversion, regeneration, sanctification, Spirit baptism, divine healing, ministry calling), and reason for applying to this graduate school. This should be typed, five pages, double-spaced with one-inch margins, and e-mailed to amd@cogop.org.
  5. Once the above is received, AMD will request recommendations from those whose names have been provided by the applicant.

Degree: Doctor of Ministry
Requirements

(one of the following):

  • Master of Divinity degree from an accredited institution.
  • A degree in Bible/Theology with completion of a portfolio.
  • A degree in a ministry related field such as counseling, education, and leadership with completion of additional seminary courses.
  1. Fill out application at cogopamd.org under the “ADMISSIONS” option; select “Doctor of Ministry Program.”
  2. Submit non-refundable $100 application fee to AMD.
  3. Request official transcript from each accredited institution of higher learning documenting the completion of a degree. Transcripts must be sent from the institutions to AMD either by e-mail (amd@cogop.org) or regular mail.
    Note: Graduates from non-American will need their transcript evaluated.
  4. Submit a 2-3 page personal narrative on your ministry call, experience, and development.
    This personal narative may also be emailed to amd@cogop.org
  5. Once the above is received, AMD will request recommendations from those whose names have been provided by the applicant.
